A Community Action ToolkitMay 7, 2007 View full document (PDF format) As part of the DIABETES AWARENESS DAYS 2007 CALL TO ACTION campaign, the Indiana Minority Health Coalition contracted the development of the Community Action Toolkit. The toolkit provides culturally specific diabetes prevention of information, education, care and treatment services to help reduce the burden of diabetes within our communities. Topics include: diabetes statistics; impact of diabetes in Indiana; practical tools to plan and coordinate local activities; as well as technical details about organizing and coordinating advocacy, educational and fundraising events. |
