LaPorte County Health Needs Assessment - Executive Summary

The purpose of the Health Needs Assessment project was to identify health care access-related issues that the minority community in LaPorte County perceives to be important and that need to be addressed. Data for the health assessment were collected from existing data containing health indicators, targeted surveys, focus groups, and key informant interviews. A brief review of the results of the assessment are described in the executive summary.
